Registration for the 2017-2018 School Year

Additional forms may be required at time of registration.
 
  One set of forms must be completed for each child being registered.
 
Physical Examinations and proof of immunizations are needed for:
  Kindergarten
  6th Grade
  9th Grade
  All students new to the district.
 
Vision Examinations are needed for:
  Kindergarten
  All students new to the district.
 
Dental Examinations are needed for:
  Kindergarten
  2nd Grade
  6th Grade
  All students new to the district.
 
  Your child (grades K, 6, 9, and all new students) must have an Illinois School Physical and all immunizations up-to-date and on file with the school office by the first day of school or your child will not be able to attend.
 

Immunization Updates:

 School Year 2016-2017 Meningitis Vaccine requirement for grades 6 and 12 

 A new vaccine is required for entry into grades six (6th grade) and twelve (12th grade) this coming school year, beginning August 2015.  This vaccine protects students against a severe form of bacterial meningitis, as well as other diseases caused by this bacterium.  Please make plans to assure that your student is protected against meningitis and ready to start school in the fall.

The Meningitis Vaccine can quickly and efficiently be administered at your local physician’s office.
